In the case of blood donation risk, the infection of blood recipients by the HIV and Hepatitis C viruses in many countries around the world, including Canada, in the 1980s was such a catastrophic failure. This risk is known as \u00e2\u20ac\u0153transfusion-transmitted infection\u00e2\u20ac\u009d (TTI).
